Description
Benzathine Benzylpenicillin 2.4 MIU Injection is a long-acting beta-lactam antibiotic belonging to the penicillin group. It is supplied as a powder for reconstitution or a pre-constituted suspension, intended for deep intramuscular (IM) injection only. Unlike other forms of penicillin that are absorbed quickly, benzathine penicillin is designed to be slowly released from the injection site into the bloodstream over an extended period (weeks). This sustained release maintains therapeutic levels of the antibiotic, making it highly effective for specific long-term treatments and prophylaxis.
The antibiotic works by inhibiting bacterial cell wall synthesis. It interferes with the last stage of peptidoglycan synthesis, which is crucial for the structural integrity of bacterial cell walls. This leads to the lysis and death of susceptible bacteria, particularly during their active growth phase. Benzathine Benzylpenicillin is highly effective against susceptible Gram-positive bacteria, notably Streptococcus pyogenes (Group A Strep) and Treponema pallidum (the bacterium causing syphilis).
Key indications for Benzathine Benzylpenicillin 2.4 MIU Injection include:
- Treatment of Syphilis: It is the primary treatment for various stages of syphilis, including primary, secondary, and early latent syphilis, as well as late latent syphilis (often requiring multiple doses).
- Prophylaxis of Rheumatic Fever: Used for the long-term prevention of recurrent rheumatic fever and rheumatic heart disease in individuals who have previously had acute rheumatic fever.
- Treatment of Streptococcal Infections: Especially certain upper respiratory tract infections (like strep throat) due to susceptible Group A Streptococci.
- Treatment of other endemic treponematoses: Such as yaws, bejel, and pinta.
Important Considerations:
- Route of Administration: STRICTLY for deep intramuscular (IM) injection only. Never administer intravenously (IV), subcutaneously (SC), or intra-arterially, as inadvertent IV administration can lead to severe and potentially fatal cardiorespiratory arrest.
- Penicillin Allergy: A thorough history of penicillin or cephalosporin allergy must be taken before administration, as severe hypersensitivity reactions (including anaphylaxis) can occur.
- Jarisch-Herxheimer Reaction: This is a common, self-limiting reaction (fever, chills, headache, myalgia) that can occur within hours of the first dose, particularly when treating syphilis, due to the rapid killing of bacteria.
- Pain at Injection Site: The injection can be quite painful due to the volume and suspension nature of the product.
